I recently went down a rabbit hole comparing what's in modern first aid kits and reading articles and reviews. After all that, I decided the best thing would be to replace the few things in my 15-year old kit that may be past their prime. I figured that would be less costly than replacing everything. It was, but not by much.
What I bought:
Diphenhydramine, 24 tablets (generic Benadryl) Loperamide, 12 caplets (generic Immodium) 6 large-ish bandages (2.38x3.5 in)
12 butterfly closures 10 1x3 in bandages + 10 3/4x3 in bandages
Total cost: just short of $31
I would put about half of each in my kit, which would make the replenishment about $15, or about 1/4 the cost of replacing the whole thing.
Despite the expense, I'll be happy if this ends up as beaten-up and "unused" as what it's replacing.

I'm not sure if it's the Diphenhydramine or the Loperamide that gets the most use in my kit. Both need to be refreshed.

I just finished a hike in AZ last week and before I went I was looking at my FAK. Its not very big but it definitely weighs more than most would put up with. I need to redo it more than I need to refresh it with newer stuff. The stuff that gets used most in mine is ibuprofen and blister care. If I get a chance maybe I'll list my FAK so you guys can make fun of it.

I use the Loperamide on almost every trip at elevation, because that’s basically the one and only travel malady that I’m very proactive about medicating at the first indication. Otherwise, I’m a pretty typical old guy: “oh, it’ll be fine—nothing’s broken, nothing’s falling off…”

The things I use most are aspirin and finger-sized bandages. Long ago I used a lot of blister stuff, but now I mostly carry it for my wife.
What prompted my refresh was noticing the how badly the packaging of the larger bandages had deteriorated.

I have the same issue with the packaging for the bandages, though I also found some of my supplies were running low or had expired.

A refresh is a very good idea: medications expire and I suspect the flimsier packaging if kit supplies exposes the drugs to even more oxygen etc. that hurts them.
